FTE: 0.5, Shift: Day, Schedule: Monday-Friday, Hours Vary

This position is within Parent to Parent, a peer support program at Children's Village. Individuals with lived experience (parents/caregivers raising children with disabilities) are encouraged to apply.

Position Summary

The Community Health Worker (CHW) assists individuals to adopt healthy behaviors. The CHW conducts outreach for medical personnel or health organizations to implement programs in the community that promote, maintain and improve individual and community health. The CHW may provide information on available resources, provide social support and informal counseling, and advocate for individuals and community health needs.

ESSENTIAL RESPONSIBILITIES
1.Provides health education, informal counseling for social support, care coordination, health services enrollment and navigation.
2.Provides guidance to ensure any needed screenings and/or agency applications are completed.
3.Enrolls and orients clients to program.
4.Educates clients to improve access to services and supports that improve health outcomes.
5.Provides clients with basic health messages.
Participates in regularly scheduled team meetings and/or case conferences to assure communication, coordination and continuity of care.

Requirements


Education:

High School diploma or GED equivalent required.

Experience:

Minimum of one year of health care or social services experience preferred.
Licenses/Certificates/Registration: Completion of Community Health Worker Training Core Competencies Certificate through the Washington State Department of Health within one year of employment required.

Pay and Benefit Expectations

We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $19.54 - $27.36 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant's years of experience align.
